COVID-19 Protocols and Questionnaire

As you may know, I have moved my massage practice to Bodyworx in Comox. Please find the information in this link about our office compliance with COVID-19 protocols. Also attached is a COVID-19 screening questionnaire. This is to be filled out just before you come in for a treatment.
